Check Transcripts

To obtain official transcripts from USM institutions, you may send a written request to the institution attended or, at some schools, you may request a transcript online. Mailing addresses for all USM schools are provided below, as well as links to online information and transcript request forms where available. Please include the following information with your request:

  • your name (during the period attended)
  • your social security number
  • your date of birth
  • dates attended
  • number of copies needed
  • the mailing address for the transcripts
  • your signature
Most institutions will process faxed requests; a fax number is indicated where this service is offered. Include the information noted above in your fax. It generally takes 3 to 5 working days to process transcript requests; at times of peak demand, it may take somewhat longer. Several of the schools offer "rush" service for a fee, and/or additional transcript services; for details, see the webpage links provided. There is no charge for transcripts unless otherwise indicated. Students/formerstudents must be in satisfactory financial standing with their school in order to receive transcripts, and the student must make the request for transcripts -- third party requests will not be honored.
